LATIS Video Services Equipment Checkout Agreement 

By making a reservation or checkout at LATIS Video Services, you agree to the following: 

  • Equipment refers to all cameras, tripods, batteries, audio/video supplements, lights, computers, editing resources, storage, A/V devices, studios, labs, studio equipment, and miscellaneous gear. 

  • All equipment must be returned by the specified time. If equipment is damaged, lost, or stolen the user is responsible for the cost of replacement or repair, including concealed or unreported damage discovered after return. Any problems with equipment should be brought to the attention of LATIS Video Services staff. In the event the value of lost or damaged equipment exceeds the cost of the University's deductible for an insurance claim, the user will be responsible for the full cost of the deductible.

  • Equipment is limited and primarily intended to support student assignments. Late or damaged returns can jeopardize fellow student's classwork.

  • LATE RETURNS will result in a $25 late fee, per checkout, per day. Each additional day past the due date will incur a further $25 per day. Fees will accrue until equipment has been returned or replaced. After an initial $25 late fee, final fees will not exceed the replacement value of unreturned equipment. Fees will be assessed directly to the user’s UMN account. Excessive late returns or failure to replace lost or damaged equipment may result in loss of access to the loaner pool. 

    A late fee will not be charged if you are present with the equipment at the scheduled return time and the return process extends beyond the scheduled return time. 

    Late fees and fees for damage or loss are considered by a committee that gives a recommendation on whether to charge a fee. You can email [email protected], call 612-624-0278, or contact the studios and equipment manager to discuss the circumstances. 

Checkouts will only be made to Authorized Users (User): 

Equipment will only be checked out to the Authorized User listed unless approved by a LATIS Video Services manager. The Authorized User must be a currently registered UMN student or University staff/faculty member. In the case of students, they must be enrolled in CLA or a CLA course, have instructor authorization, or be a LATIS employee. Equipment must be in the possession of the Authorized User at all times. Additional operators may use the equipment within the presence of the Authorized User. A guest operator is the responsibility of the Authorized User. Responsibility for any lost or damaged equipment while in the care of a guest user falls upon the Authorized User. 

Equipment for University purposes only: 

Equipment is only to be used for the purposes of University of Minnesota student work, or other University business. Equipment may not be used for any private business, nor used in any manner to cause harassment, destruction of or damage to property, software, or data belonging to others, nor for unauthorized copying of copyrighted materials, or any other unauthorized use. 

Return equipment in same condition: 

The Authorized User shall return equipment to LATIS Video Services in the same condition they received it, excluding standard wear and tear, to LATIS Video Services Equipment Checkout Center, located in 640 Rarig Center. The Authorized User will be held responsible for equipment returned by someone other than Authorized User. Equipment will not be considered returned until it is inspected and returned by a LATIS Video Services employee. If the equipment is damaged or lost, the Authorized User will be responsible for the cost of repair or replacement. In the event the value of lost or damaged equipment exceeds the cost of the University's deductible for an insurance claim, the user will be responsible for the full cost of the deductible.

Waiver for incidental, special, or consequential damages to you: 

Authorized User waives all claims of damages or harm to the user(s) against LATIS Video Services, the College of Liberal Arts and the University of Minnesota for any incidental, special, or consequential damages in connection with furnishing, use of, or performance of equipment. 

Do not repair or alter equipment: 

The Authorized User will not attempt any repairs to the equipment, or attempt to replace any part of the equipment. The Authorized User will not tamper with or alter equipment. The Authorized User will pay for all unauthorized repair costs and replacement parts, as well as the cost of restoring any unauthorized alterations. 

Accidental loss or theft: 

In the event equipment is stolen or lost due to no fault of the user, the Authorized User will promptly complete incident reports and deliver to LATIS Video Services a copy of all related documents. The Authorized User will fully cooperate with LATIS Video Services’ investigation of any vandalism, theft, accident, claim, or lawsuit involving use of equipment, which may involve law enforcement. 

If equipment has been stolen or vandalized, the Authorized User should file a police report as soon as the theft is discovered. The Authorized User should notify LATIS Video Services immediately after filing a report and provide LATIS with a copy of the report. 

Optional Insurance: 

There are optional, external insurance opportunities that may cover loss or damage. Depending on the policy, sometimes homeowners or renters insurance can cover claims. There are also companies offering college insurance for students borrowing high value items. We recommend an internet search for “College Insurance.” These options are not affiliated with the University of Minnesota in any way.
